Overview
MnCHOICES assessment is a free service from the county to help people and their families plan and choose the right services. A social worker will visit the person’s home to do the assessment with them and their family. After the visit, the assessor will give information and guidance based on the person’s needs and what programs they qualify for.
Together, the person, their family, and the assessor can talk about care plan options, keeping in mind what matters most to the person and their unique situation.
An assessment can also be done if someone is living in a nursing home and thinking about moving back into the community.
