Central MN Alliance (CMA)

The Central Minnesota Alliance (CMA) is a partnership between Benton, Sherburne, and Stearns Counties and CentraCare, established in 2018 to strengthen regional community health planning. CMA partners work together to complete community health assessments and improvement plans.

What is the Community Health Survey?

The Community Health Survey gathers data about the health of adults across the region. Surveys were conducted in 2013, 2016, 2021, and most recently in 2024. In 2024, a convenience electronic survey was conducted across all three counties.

What is the Community Health Assessment & Improvement Plan (CHNA & CHIP)?

Every five years, public health agencies are required to complete a Community Health Needs Assessment (CHNA) and develop a Community Health Improvement Plan (CHIP). These efforts identify health priorities and guide coordinated community action.
